These are the steps to complete an insurance claim for a person injured

• Athlete and Club Complete the VSA Approved Incident Report here
• The Club / Injured Person with the assistance of the player submit the claim details on the online form (here)

Volleyball South Australia will only action if the above steps are completed by the club and/or injured person.

For Volleyball SA run competitions the above steps will need to be completed by a staff member, supplier and/or injured person. Club on the form will need to be selected as Volleyball SA.

Sportscover will then reach out to Volleyball South Australia for confirmation of membership and level of insurance.
